Adult Education Director Phyllis Butland was awarded the CTA Southeastern Service Center Council WHO (We Honor Ours) Award. She is serving her fourth year on the Executive Committee and is well known for her lobbying efforts on behalf of Adult Education. Phyllis will be honored at a banquet at the Cerritos Sheraton the evening of Wednesday, June 9.

Designate a Physician for Workers’ Compensation
In order to ensure that you will be eligible to be treated by your personal physician during a workers’ compensation, employees may complete the Designate a Physician form. The Emergency Information Card is not the Designate a Physician form and cannot be substituted for the appropriate form. Without the completed form received by Risk Management prior to a workers’ compensation claim, the District will require you to receive medical treatment from a District selected physician.
The Designate a Physician form is available through Risk Management (extension 7919) or your site secretary. The form will remain current with Risk Management for the duration of your employment with the District. Should you want to change your designated physician contact Risk Management.
Consider the Possibilities · Apply for a Sabbatical
If you have 7 years of consecutive service in the District, you are eligible for a sabbatical. Applications will be considered for the purpose of travel, projects, and/or study which will benefit the schools and pupils of the District. Sabbatical may be for one or two semesters.
Applications are available from Human Resources at the District Office. Applications must be received no later than 4:30 p.m. Thursday, January 15.
